MediHealth Connect helps NDIS participants across South East Queensland navigate these transitions with the right planning, the right supports, and the right connections in place before change happens.
Under the NDIS, life stages and transition supports help participants manage significant changes in their lives. This is funded under Capacity Building — specifically "Assistance in Coordinating or Managing Life Stages, Transitions and Supports."
The NDIS recognises that transitions are high-risk moments for participants. Without proper planning and support, gaps in services can open up, and progress made over years can be lost during a poorly managed transition.
Leaving school is one of the biggest transitions for young people with disability. We help with exploring post-school options (employment, further education, day programs), connecting with disability employment services and supported employment providers, building workplace skills and confidence, and coordinating with schools during the transition planning process.
Whether a participant is moving into supported independent living (SIL), shared housing, or their own place, this transition requires careful planning. We help identify housing options, set up daily living supports, coordinate with accommodation providers, and ensure continuity of services during the move.
Changing providers — whether it's moving to a new support coordinator, a different nursing provider, or a new support worker team — needs to be managed carefully. We coordinate handovers, transfer relevant information (with consent), and monitor the transition to make sure nothing falls through the cracks.
As participants age, their needs often change. They may need more support with daily living, different types of clinical care, or access to aged care services alongside their NDIS supports. We help navigate the intersection between NDIS and aged care, coordinate with relevant services, and adjust support plans as needs evolve.
Being discharged from hospital can be a vulnerable time. We coordinate with hospital discharge teams to make sure supports are in place before you leave — community nursing, daily living support, transport, and any equipment or modifications needed at home.
Plan ahead. Transitions work best when they're planned well in advance. We start the conversation early, identify what needs to happen, and create a timeline that allows for proper preparation.
Coordinate everything. Transitions usually involve multiple providers and systems. We act as the coordination point, making sure everyone is communicating and the participant isn't lost in the handover.
Monitor the landing. We don't just hand off and walk away. After a transition, we check in to make sure everything is working as planned and adjust if it's not.
Ideally, planning should begin at least 12-18 months before the young person finishes school. This gives enough time to explore options, trial different settings, and set up post-school supports.
Yes. When participants reach 65 and transition from the NDIS to aged care, the process can be complex. We help coordinate between the two systems to ensure continuity of support.
Transition support is typically funded under Capacity Building in your NDIS plan. The specific funding depends on the type of transition and your assessed needs.
