Agreement to Terms

By engaging MediHealth Connect for NDIS community nursing or support coordination services, you agree to the following terms and conditions. These terms should be read in conjunction with your individual Service Agreement.

Our Services

MediHealth Connect provides NDIS community nursing care and support coordination services to eligible NDIS participants across South East Queensland. All services are delivered in accordance with the NDIS Practice Standards, the NDIS Code of Conduct, and applicable Australian laws.

Service Agreements

Before services commence, we will enter into a Service Agreement with each participant. The Service Agreement outlines the specific services to be provided, the schedule, pricing, responsibilities of both parties, and the process for making changes or ending the agreement.

NDIS Pricing

All services are charged in accordance with the NDIS Price Guide and Support Catalogue current at the time of service delivery. We do not charge above the NDIS maximum price for any service.

Cancellations

If you need to cancel a scheduled service, we ask for at least 48 hours' notice where possible. Cancellations with less than 48 hours' notice may be charged in accordance with the NDIS cancellation policy.

Short-notice cancellations and no-shows may be charged at 100% of the agreed service price, as permitted under the NDIS Price Guide.

Limitation of Liability

While we strive to provide the highest quality services, MediHealth Connect is not liable for circumstances beyond our reasonable control, including service disruptions caused by natural disasters, pandemics, or other force majeure events. Our liability is limited to the extent permitted by law.

Governing Law

These terms and conditions are governed by the laws of Queensland, Australia. Any disputes will be subject to the jurisdiction of the courts of Queensland.

Changes to These Terms

We may update these terms from time to time. The most current version will always be available on our website. Material changes will be communicated to active participants.
