We coordinate with your Brisbane care team. If your GP is in Chermside, your specialist is at the PA Hospital, and your pharmacist is in Sunnybank, we'll communicate with all of them. Good care depends on everyone being on the same page.

1. Reach out. Contact us directly, or ask your support coordinator to make a referral. We'll have a quick conversation about your needs and NDIS plan.
2. In-home assessment. A registered nurse visits you at your Brisbane home to carry out a clinical assessment. They'll review your health, discuss your goals, and develop a care plan.
3. Care begins. Once the plan is agreed and funding is confirmed, your nurse starts visiting on the schedule that works for you.
The whole process from first call to first nursing visit typically takes about one to two weeks, depending on the urgency of your needs.
Community nursing is funded under the Capacity Building section of your NDIS plan. If nursing is already in your plan, you can choose us as your provider and get started straight away.
If nursing isn't currently in your plan but your health needs have changed, you can request a plan review. Your GP will need to provide supporting evidence, and your support coordinator can help you through the process.

For standard nursing needs, we can usually begin care within one to two weeks of your initial enquiry. Urgent or high-priority cases may be seen sooner — let us know your situation and we'll do what we can.
Absolutely. We work alongside your GP, specialists, allied health professionals, and support coordinator. Community nursing complements your existing care — it doesn't replace it.
Our services are designed for community settings — your home, supported living, or community locations. We don't provide in-hospital care, but we can step in as soon as you're discharged to continue your recovery at home.
