NDIS Wound Care & Medication Management

Wound Care Services

Wounds that aren't properly managed can deteriorate fast. What starts as a minor pressure area can become a serious infection. What should have been a straightforward surgical recovery can drag on for months.

Our wound care nurses assess each wound individually, identify the right treatment approach, and carry out dressing changes using evidence-based protocols. We don't guess — we document, measure, and track progress so your treating doctor has clear information to work with.

Types of Wounds We Manage

Our Approach to Wound Care

Every wound gets a proper assessment before we start treatment. Your nurse will photograph and measure the wound, assess the tissue type, check for signs of infection, and develop a dressing plan. This plan is shared with your GP and reviewed at every visit.

If a wound isn't responding to treatment as expected, we escalate. That might mean a referral to a wound care specialist, a change in dressing products, or a review of underlying factors — like nutrition, medication, or circulation — that could be slowing healing.

We also educate participants and carers on basic wound care between nursing visits. Things like keeping dressings dry, recognising signs of infection, and knowing when to call us can make a significant difference in outcomes.

Medication Management

Managing multiple medications is complicated, and the stakes are high. Missed doses, wrong combinations, or incorrect timing can lead to hospitalisations. Our nurses take the guesswork out of medication management for NDIS participants.

What Medication Management Includes

Why Medication Management Matters

The reality for many NDIS participants is that they take multiple medications prescribed by different doctors. Without proper oversight, things slip through the cracks. A new medication interacts with an existing one. A dosage change at the hospital doesn't get communicated to the community pharmacy. A support worker isn't sure whether the PRN medication is appropriate right now.

Our nurses act as the clinical link between your prescribers, your pharmacist, and your day-to-day care team. We make sure everyone is on the same page.

Continence Assessment and Management

Bladder and bowel control issues are more common than people think among NDIS participants, and they shouldn't be something you just put up with. Proper assessment and management can dramatically improve quality of life.

Our continence assessments cover:

We approach continence with the same professionalism as any other clinical need — no awkwardness, no judgement, just practical care that makes your day easier.

Palliative Nursing Care

When a participant is living with a life-limiting illness, comfort and dignity become the priority. Our palliative care nurses provide:

We work alongside your existing palliative care team to provide the in-home clinical support that allows you to remain comfortable in familiar surroundings.

Frequently Asked Questions

How often will a nurse come for wound care?

It depends on the wound. Fresh surgical wounds or infected wounds may need daily dressing changes. Stable chronic wounds might only need attention two or three times a week. Your nurse will assess and recommend the right frequency.

Can your nurses set up my Webster pack?

We work with your pharmacy to make sure your Webster pack is accurate and up to date. Our nurses verify the contents, administer medications, and flag any discrepancies with your pharmacist and GP.

Do you provide continence products?

We carry out the assessment and recommend appropriate products. Ordering and supply of continence aids may be covered through the NDIS Continence Scheme or through your NDIS plan, depending on your situation. We can guide you through the options.

Is palliative nursing covered by the NDIS?

Palliative care can be complex because it may involve both NDIS and mainstream health funding. Our team can help you understand which services fall under your NDIS plan and which are covered by the public health system.
