Step 1: We acknowledge your referral within one business day.
Step 2: We contact the participant (or their family) to introduce ourselves and gather any additional information.
Step 3: We arrange an assessment — a clinical nursing assessment for nursing referrals, or an initial meeting for support coordination referrals.
Step 4: We develop a care plan or coordination plan and begin delivering services.
We keep the referring party informed throughout the process (with the participant's consent) so you know the referral has been actioned.
