Our registered nurses deliver in-home clinical care including wound management, medication administration, continence support, chronic disease monitoring, and high-intensity nursing for complex needs. We cover all of South East Queensland with local nursing teams in each region.
Our exercise physiologists and personal trainers design NDIS-funded fitness programs tailored to your abilities and goals. Whether you're building strength after an injury, managing a chronic condition through movement, or working towards greater physical independence, we create programs that meet you where you are.
MediHealth Connect connects participants with allied health professionals including occupational therapists, physiotherapists, speech pathologists, psychologists, and dietitians. We coordinate these services as part of your broader care plan so everything works together.
Our support coordinators help you understand your NDIS plan, find quality providers, coordinate your services, and build independence. We offer all three levels — Support Connection, Coordination of Supports, and Specialist Support Coordination.
MediHealth Connect's plan management service handles the financial side of your NDIS plan. We pay your provider invoices, track your budget, process claims, and give you clear financial reports — so you can focus on your life, not paperwork.
For participants with behavioural support plans, we provide trained staff who can implement strategies consistently and safely. Our team works alongside behaviour support practitioners to put plans into practice and monitor outcomes.
Major life transitions — leaving school, starting work, moving out of the family home, ageing — require careful planning and the right support. We help participants navigate these changes with coordination, skill-building, and provider connections.
From personal care and meal preparation to community access and social participation, our support workers help you with the daily activities that keep life moving. We match you with consistent, trained workers who respect your preferences and routines.
Getting where you need to go matters. We provide transport assistance to medical appointments, therapy sessions, community activities, and social events. Our transport support is flexible and arranged around your schedule.
For young children with developmental delays or disabilities, early intervention can make a lasting difference. MediHealth Connect provides early childhood supports that help children build skills during the critical early years.
We help with household tasks like cleaning, laundry, and gardening, as well as coordinating home modifications — ramps, bathroom rails, kitchen adjustments — that make your home safer and more accessible.
For participants and families from culturally and linguistically diverse backgrounds, communication matters. We arrange interpreting and translating services so language is never a barrier to accessing your NDIS supports.
